Insurance Information
To use insurance for your treatment, please bring your current insurance card and a valid picture ID with you. The following forms of identification are accepted:
- Driver’s license
- Government-issued identification card
- Passport

Please check with your insurance carrier directly to verify that we are contracted provider with your insurance.
If you have an HMO insurance it is your responsibility to ensure you have the proper authorization and/or referral in advance of your appointment to avoid having to reschedule or self pay for your appointment.
Medicare
We see many Medicare patients. However, we do not accept assignment. This means the Medicare process works differently at our office.
Payment is due at the time of service. We will submit your claim and any reimbursement will be sent directly to you. If you have a secondary insurance or supplement, you will need to check with Medicare if they apply.
Minors and Patients with an Assigned Guardian
Patients under the age of 18 and patients with an assigned guardian must be accompanied by a parent, legal guardian, or power of attorney at each appointment. The parent or legal guardian accompanying a patient will need to show their photo ID and is responsible for providing the most recent insurance information and completing the appropriate paperwork for the patient.
